Report on August 6 Helicopter Crash Submitted to President Mahama

The committee investigating the August 6, 2025, military helicopter crash has submitted its final report to President John Dramani Mahama. The report was presented on Monday, November 10, during a National Security Council meeting at the Jubilee House in Accra by Acting Minister for Defence, Dr. Cassiel Ato Forson, who chaired the committee.

The investigation team was tasked with uncovering the cause of the fatal crash and recommending measures to prevent future aviation disasters.

According to government sources, the report includes comprehensive findings on technical, operational, and environmental factors that may have contributed to the tragedy.

The government has announced that the report’s findings will be made public on Tuesday, November 11, 2025, during an official press briefing, where recommendations and next steps will be shared.

The incident involved a Harbin Z-9EH military helicopter (tail number GHF 631) belonging to the Ghana Air Force. The aircraft departed from Accra at around 9:12 a.m. for an anti-illegal mining operation in Obuasi but lost radar contact and crashed in the Adansi Akrofuom District.

Tragically, all eight passengers and crew on board lost their lives. The victims included: Dr. Edward Omane Boamah, Minister for Defence; Ibrahim Murtala Muhammed, Minister for Environment, Science and Technology; Muniru Mohammed Limuna, Acting Deputy National Security Coordinator; Samuel Sarpong, NDC Vice-Chair; Samuel Aboagye, former parliamentary candidate; Squadron Leader Peter Bafemi Anala; Flying Officer Manaen Twum Ampadu; and Sergeant Ernest Addo Mensah.

The investigation, led by National Security Coordinator Abdul-Osman Razak, was conducted with support from the Ghana Air Force, the Ghana Civil Aviation Authority (GCAA), and international aviation experts.

The final report is expected to provide vital insights into the root causes of the crash and guide future policies to enhance flight safety in Ghana’s military operations.
